There is no doubt that health care delivery is rapidly changing. The Affordable Care Act (ACA) has survived Supreme Court challenges and appears to be here for the long term. A central feature of this act is the formation of Accountable Care Organizations (ACO) and Patient Centered Medical Homes (PCMH). It is the PCMH that will dramatically change the market for health care services. This course begins with economics. Market behavior depends on supply and demand. While the ACA will significantly alter supply and demand of medical services, PCMHs will change how those services are accessed by the public. This course will review the economics of the ACA, the effect of PCMH on patient’s access to your practice, and then the \what to do\” in response. The ACA and PCMH will affect every physician in every specialty in every setting.

All physicians have capital requirements in their practice. Whether it is hiring a new employee, purchasing a new piece of equipment or updating/starting a new service line, it all takes money. Yet while the ins and outs of capital have not been part of our medical education we are still required to be the main decision makers. How do we usually deal with this? We don’t. We outsource these decisions to our administrative personnel who may or may not know any more than we do. Part 1 continues your finance education and allows you to learn the basics of where to get money, how much debt is good and the cost of capital. Part 2 tells you how to spend it appropriately and outlines the steps to craft a proposal for equipment that you need, or to make decisions about capital spending in your organization.

The most frustrating thing about HIPAA is that it has never been explained properly. HIPAA actually improved patient related communication by placing everyone under the same set of simple federal rules. Despite all that has been said, HIPAA is surprisingly conducive to the practice of medicine and difficult to violate with anything short of unprofessional conduct. This presentation evaluates the common HIPAA dilemmas – talking with family members, leaving messages on answering machines, communicating confidentially in a busy emergency room, overheard conversations, pharmaceutical representatives in the office, announcing patient names in a waiting room, the use of patient sign-in sheets, a patient’s right to access and alter his/her record, and handling insurance company pre-authorizations.

Iron presents a problem for the body. On the one hand it is absolutely essential to life. It is at the center of the oxygen carrying capacity of hemoglobin as well as cytochromes that provide energy for cells. But at the same time, iron can be toxic to cells; oxidizing lipids in membranes and proteins. Thus the body is challenged to regulate iron in a way that provides for essential needs, maintains a ready store for emergencies, and yet avoids toxic overload. This self-study will review the regulatory and storage mechanisms for iron metabolism, discuss diseases in which iron regulation is disturbed, and the testing performed in the clinical laboratory for the assessment of iron.

Contrary to common belief, good leadership is a learned quality. Character traits of charisma and personal magnetism are not the same as good leadership. All physicians are leaders in the workplace of their practice. It doesn’t matter whether you are in academics, employed by a hospital, or in private practice. The fact is, you lead every day! Others look up to you and expect direction. The manner in which you provide leadership will impact on your ability as a physician and eventually affect the quality, safety and cost of the care you deliver to your patients. Would you like to become a better leader?

Infectious diarrheal disease is a major cause of morbidity and/or mortality in the world, resulting in many physician office visits and hospitalizations per year. Many of the agents involved are bacterial in origin, especially Gram negative rods belonging to the family Enterobacteriaceae. Some microbes that cause diarrheal disease may reside in the intestinal tract as normal flora, where they cause disease only under special circumstances. Other agents are not a part of the normal intestinal flora but rather cause infection and invade via exogenous exposure by ingestion of contaminated food or water, or by direct contact with an infected individual. This course examines several pathogenic serotypes of Escherichia coli, and includes a discussion of the potential sources of infection; the typical clinical presentations associated with this pathogen as well as a brief discussion of pathogenic mechanisms.

Virology is one of the newer diagnostic microbiology subspecialties, as viruses and the means to cultivate them were discovered well after the characterization of bacteria and other microorganisms. During its relatively short history, much has changed in clinical virology. Many new viruses and viral diseases have been identified. Much has been learned about the properties of viruses and the pathogenesis of viral infection. Diagnostic virology has evolved from an esoteric into an important routine laboratory service as effective antiviral agents have become available and testing has become faster and more accurate. To a large extent, this is the driving force behind the evolution of diagnostic clinical virology. The clinical virology laboratory is now able to provide diagnoses that impact patient management and treatment decisions and public health interventions.

Even though these ions are grouped together for analysis or study, each plays a distinct role in the body. Therefore, there is much to know about each ‘s physiology and the effects that increased or decreased levels in the blood may have on a patient’s condition. This electrolyte self-study will help the participant gain a useful understanding of the most common electrolytes, the role that water plays in maintaining electrolyte balance in the body, and how osmolality is used as a measure of serum or urine concentration. In addition, the specific regulation and functions of each electrolyte, the usefulness of the anion gap, and the significance of urine electrolyte levels is included. Finally, the most common methods of analysis are presented.”

Board certification is a requirement in the credentialing process for many hospital systems. No matter what the medical specialty, major change has come to the certification process. Since 1990 most physicians have been subject to recertification which included continuing medical education and periodic testing components. A four part process is now the standard in all medical and surgical specialties. Although the testing component remains relatively unchanged, the education component has a different face and a practice performance part has been added. The goal of this course is help physicians navigate recertification. Knowing what to do and why you are doing it, changes the process from a nuisance into an opportunity for each physician to maintain and improve the care they give to their patients.

More than 50 percent of all physicians participate in the Medicare Electronic Record Incentive Program better known as Meaningful Use. Who in your practice is responsible for the decision on whether to participate or, for that matter, the implementation of the data collection and reporting if you are already registered? No matter how large your practice is, the answer to that question should be YOU. Meaningful Use is not about other physicians in your group, your business manager or your IT vendor. It is about you and Medicare. If you plan to stay enrolled in Medicare, familiarity with Meaningful Use is essential!

2014 is the last year to qualify for an incentive to report quality measures to Medicare. Who can afford not to accept an extra half percent bonus on all of your Medicare payments? Half percent, not that big a difference? If you don’t participate? Physicians suffer a 1.5% penalty. All of a sudden that half percent turns into a 2% swing resulting in a 3 – 5 thousand dollar loss per physician for most practices. Is the PQRS program more trouble than it is worth? For the uninformed the answer is absolutely yes!

In this course we will discuss appropriate procedures for the verification and validation of FDA-approved (-cleared) laboratory assays to include diagnostic tests, microbial identification /susceptibility tests, and blood culture instrumentation. The most important attributes of a laboratory test are its ability to produce accurate, precise and reproducible results with a rapid turn-around-time. Thus, laboratories are required to establish the performance characteristics of new assays and to provide policies and procedures to maintain reliability, efficiency, and clinical utility of laboratory tests; this is accomplished by verification and validation testing.

A survey conducted by Health Information Management Association (AHIMA) June 2015 demonstrated 50% of hospitals and 17% of physician practices had conducted ICD 10 diagnosis and procedure code testing. Only 6% of primary care physicians have participated in testing. The lack of preparedness reflects the inadequate buy-in from physicians who believe it is a useless transition that will result in the collection of large volumes of data that will eventually be used against them. (Healthcare Informatics 4/15). Two things are clear. First, ICD – 10 is here to stay and will be the rule of law on Oct 1, 2015. Second, physicians don’t understand the what, why and how to of the new system. The goal of this CME course is to fill in the blanks of ICD – 10.
